Build a third-party assessment template

A template is the questionnaire you send to a third party. Each question carries a risk weight, and the answers produce the vendor's risk score.

Where: Trust Management → Third-Party Risk Management → Templates, or open Templates.

Who: admins, or members whose department has access to the TPRM module.

1. Create the template

  1. Open Templates and click Create Template.
  2. Enter a Name, choose or type a Category (typing a new name offers Create "…"), and add a Description.
  3. Click Create Template. The template opens in the builder as a draft.

Already have your questions in a spreadsheet? See Import a template from Excel or CSV.

2. Add questions

  1. Click Add New Question.
  2. Write the Question Text, for example "Do you hold a current SOC 2 Type II report?".
  3. Pick the Answer Type: Text input, Text area, Radio button, Checkbox, Dropdown menu or File.
  4. Turn on Required question if the vendor must answer it before submitting.
  5. Set the Risk Weight: how many of the template's 100 risk points this question can add. Each question can carry 0 to 40 points; 0 means the question is asked but never affects the score.
  6. For Radio button, Checkbox and Dropdown menu, add at least two Answer options with Add Option and give each a Score from 0% (no risk) to 100% (maximum risk). For Checkbox questions the option scores must add up to 100.
  7. Optional: under Dependencies, click Show logic and Add condition to show the question only when an earlier question has a given answer.
  8. Click Create question.

The weight bar above the Questions List shows how many of the 100 points are allocated.

3. Publish

  1. Make sure the question weights total exactly 100. Until then Publish is disabled and its tooltip says "Weights must total 100. They currently total N."
  2. Click Publish.

Only published templates can be sent to third parties. See Send an assessment to a third party.

Change a published template

Click Edit on a published version to start a new draft. Assessments already sent keep the version they were sent with. Use the version selector in the builder to switch between versions.
